Immunocore offers the KIMMTRAK CONNECT program to deliver customized support for patients who have been prescribed Kimmtrak (Table).

KIMMTRAK CONNECT

KIMMTRAK CONNECT is a support program available at no cost for adult patients receiving Kimmtrak infusion for the treatment of metastatic uveal melanoma. The program matches patients and caregivers with a dedicated nurse case manager who can provide customized support—from financial assistance to scheduling appointments to educational support.

KIMMTRACK CONNECT Co-pay Support Program

Patients may be eligible for the Co-pay Support Program if they:

• Are prescribed Kimmtrak for an FDA-approved indication
• Are aged ≥18 years
• Are a resident of the United States or Puerto Rico
• Have commercial insurance coverage
• Do not have Medicare, Medicaid, or any other federal or state healthcare program.

Government Insurance/Uninsured/Underinsured

For your patients with governmental insurance such as Medicare, Medicaid, or TRICARE, or for patients who are uninsured or underinsured, KIMMTRAK CONNECT nurse case managers can assist in connecting patients with the financial resources for their unique situation.
