UroGen Pharma offers the UroGen Support program to assist patients while taking Jelmyto (Table).

UroGen Support

This program makes the access, reimbursement, and coordination process as simple as possible for you and your patients by providing support through territory business managers, field reimbursement managers, and clinical nurse educators. UroGen Support is committed to helping patients access Jelmyto through the UroGen Support Copay Program and the UroGen Support Patient Assistance Program.

UroGen Support Copay Program

Patients with commercial health insurance may be eligible to receive Jelmyto for as little as $50 per dose with a maximum benefit of $4000 per dose up to $13,800 per year. Patients may be eligible if they:

  • Are enrolled in the UroGen Support program
  • Have private insurance
  • Do not participate in a federal- or state-funded insurance program, including Medicare, Medicaid, TRICARE, Veterans Administration, a state prescription drug program, or the Government Health Insurance Plan available in Puerto Rico.

UroGen Support Patient Assistance Program

Uninsured patients may be able to receive Jelmyto through the UroGen Support Patient Assistance Program. Patients may be eligible if they:

  • Have no insurance
  • Have been prescribed Jelmyto for an on-label indication
  • Have an adjusted gross income equal to or less than 400% of the federal poverty level based on household income.
