Daiichi-Sankyo and AstraZeneca Enhertu4U Support Program
The Enhertu4U support program from Daiichi-Sankyo and AstraZeneca provides access and support services to assist patients who are having financial difficulties with paying for their oncology drug Enhertu (fam-trastuzumab deruxtecan-nxki) for injection.
Enhertu is indicated for the treatment of adults with unresectable or metastatic HER2-positive breast cancer who have received ≥2 regimens targeting HER2-positive disease in the metastatic setting (Table).
Enhertu4U Support Program
The Enhertu4U support program offers several services for patients who have been prescribed Enhertu, including the Enhertu Patient Savings Program, Enhertu Patient Assistance Program, and the Free Limited Supply Program. These programs may facilitate patient access to Enhertu by helping with insurance coverage and reimbursement, copay assistance, and free medication.
Patients enrolled in Enhertu4U may have access to care coordinators who are assigned by geographic region, as well as field reimbursement managers who can provide additional assistance by verifying patients’ insurance coverage. The program also provides nursing support to patients to help them throughout their entire treatment duration.
The Enhertu4U program offers reimbursement and payment support services, including benefit verification, assistance with prior authorizations, pharmacy research and coordination, claims, and billing.
In addition, the Enhertu4U program provides referral to independent foundations that offer a variety of services, such as support with copay, transportation, insurance premium, and patient education.
Enhertu Patient Savings Program
Eligible patients with commercial insurance may pay as little as $0 per Enhertu prescription, up to $26,000 annually. The annual benefit can be used to cover the cost of Enhertu, and may also pay up to $100 of the infusion cost per eligible infusion of Enhertu (residents of Massachusetts, Michigan, Minnesota, or Rhode Island are ineligible for infusion assistance).
Enhertu Patient Assistance Program
Eligible uninsured, underinsured, or Medicare patients who are having financial difficulty may receive Enhertu at no cost for up to 1 year.
Free Limited Supply Program
The Free Limited Supply Program may provide eligible patients with Enhertu at no cost if they have a coverage delay of more than 5 business days.
Eligibility
Enhertu Patient Savings Program
To be eligible for this program, patients must:
- Have a valid prescription for Enhertu
- Be a resident of the United States or Puerto Rico
- Have commercial insurance that does not cover the full cost of Enhertu
- Not be covered by state or federal insurance, including Medicare Part B or Part D, Medicaid, Medigap, Veterans Affairs, or TRICARE.
This program is applicable for claims or transactions that occur within 120 days of the date of service. This program has no income requirements for patients.
Enhertu Patient Assistance Program
To be eligible for this program, patients must:
- Not have insurance that covers Enhertu, including private or government insurance (excluding Medicare)
- Not be receiving any other help paying for Enhertu
- Have a valid prescription for Enhertu
- Be a resident of the United States
- Meet certain financial criteria.
For Medicare beneficiaries to be eligible for the Patient Assistance Programs, they must not qualify for or be enrolled in the low income subsidy for Medicare Part D, and they must have spent at least 3% of their annual household income on prescription medicines in the current year.
Free Limited Supply Program
To be eligible for a limited supply of Enhertu, patients must have:
- A coverage delay of more than 5 business days
- A valid prescription for Enhertu.

Patient Savings Program
Providers can enroll their patients in the Patient Savings Program via the Patient Savings Program Enrollment Portal for Enhertu, or by calling 833-364-3788. Patient Assistance Program
To determine eligibility for the Patient Assistance Program, patients may complete and sign the enrollment form with their provider, and fax it to 833-904-1851, or mail it to the address on the form.
Free Limited Supply Program
To enroll in the Free Limited Supply Program and access-related support services, patients and providers must complete and sign the enrollment form, and fax it to 866-760-5917.
