Puma Patient Lynx provides a complete support services package for patients who have been prescribed Nerlynx (neratinib), to facilitate easy access and affordability.

Nerlynx is indicated for the extended adjuvant treatment of adult patients with early-stage HER2-overexpressed/amplified breast cancer after adjuvant trastuzumab-based therapy (Table).

Puma Patient Lynx Services

Puma Patient Lynx provides several support services for patients who have been prescribed Nerlynx, including:

  • Financial assistance for patients who are uninsured or underinsured in finding alternate sources for financial support, if necessary
  • Benefits verification and prior authorization assistance, via reimbursement counselors who can conduct a benefits investigation, verify insurance approval or coverage, and obtain and submit necessary do
cumentation for patients requiring prior authorization.

In addition, patients who are currently using Nerlynx can access the Puma Patient Lynx medication support that provides education and side-effect counseling through the Puma Patient Lynx specialty pharmacy network, to help patients better understand and manage their therapy.

A text message support program allows patients to sign up to receive regular text messages for medication reminders and motivational information to support treatment continuation.

Puma Patient Lynx offers 3 programs for patients who are using Nerlynx.

The Patient Assistance Program provides free medications to eligible uninsured patients who meet certain financial qualifications.

The Puma Patient Lynx Co-Pay Savings Program provides copay assistance for eligible, commercially insured patients.

Nerlynx Quick Start provides a free, 3-week supply of Nerlynx to eligible patients who are experiencing delays in accessing Nerlynx.

Eligibility

Puma Patient Lynx Co-Pay Savings Program

Eligible patients treated with Nerlynx may pay as little as $10 per prescription. To enroll in this program, patients must:

  • Reside in the United States or Puerto Rico
  • Have commercial insurance
  • Not be covered under Medicare, Medicaid, or any other federal or state program
  • Have a valid prescription for Nerlynx for an FDA-approved ­indication
  • Agree to not seek reimbursement for Nerlynx from a third-party payer
  • Complete and submit an enrollment form.

Nerlynx Quick Start

Eligible patients who are experiencing a delay in obtaining coverage for Nerlynx through their health insurance plan can receive a 3-week supply of the drug for free. If a gap in coverage extends beyond the first 3 weeks and the patient and provider are actively pursuing coverage through prior authorization, the patient may be eligible to receive one 21-day refill. To be eligible for Nerlynx Quick Start, patients must:

  • Reside in the United States or a US territory
  • Have insurance coverage
  • Have a valid prescription for Nerlynx for an FDA-approved indication
  • Complete and submit the Puma Patient Lynx enrollment form.

Puma Patient Assistance Program

Patients prescribed Nerlynx may be eligible to enroll in the Patient Assistance Program if they meet eligibility criteria, including that they:

  • Reside in the United States
  • Do not have insurance or any coverage for Nerlynx
  • Are not eligible for public health insurance programs
  • Complete and submit the Puma Patient Lynx enrollment form.

Application & Enrollment

Puma Patient Lynx

To enroll in Puma Patient Lynx, patients and their providers must complete the enrollment form, and fax the completed form and any supporting documents to 844-276-5153.

The enrollment form for Puma Patient Lynx includes a section that can be filled out to request benefits verification and prior authorization assistance.

Patient Assistance Program

Completion of the Puma Patient Lynx enrollment form, including financial verification, is required for enrollment in the Patient Assistance Program.

Nerlynx Quick Start

To enroll in Nerlynx Quick Start, patients and their providers should complete the appropriate section of the Puma Patient Lynx enrollment form.

Puma Patient Lynx Co-Pay Savings Program

To enroll in the Puma Patient Lynx Co-Pay Savings Program, patients must complete and submit the online enrollment form, to have their eligibility determined. Copay assistance will be offered through the patient’s specialty pharmacy.
